Dear Matteo Thank you for your enquiry concerning postgraduate medical education in the UK.The medical training programme in the UK has changed with effect from August 2005.One of the fundamental changes to postgraduate medical training is the introduction of Foundation Programmes. All UK medical graduates will enter a two-year Foundation Programme immediately following graduation. The first year, F1 is the PHRO / internship year where doctors are required to work under Provisional registration.A new specialty training structure is due to start in August 2007. Recruitment to the new "run through" programmes is scheduled for early in 2007 and in this transition year there will be entry points at different levels to allow doctors to compete with those having similar levels of experience. In England, there will be an electronic application system (Medical Training Application Service - MTAS) www.mtas.nhs.uk/ Doctors who are unsuccessful in getting a training post leading to CCT can apply for fixed term specialist training appointments. These FTSTAs are likely to be one-year posts and have an element of training mirroring the first two years of specialty training. There will also be career grade posts for doctors with suitable experience.Further details are available on www.mmc.nhs.uk/pages/specialtiesNational person specifications are being drawn up for three different entry levels (ST1, 2 and 3) for each specialty. When they are published, you will need to compare your training and competence against these. An ST1 post will require a doctor to have achieved the competences from the Foundation Programme. You can find these in the curriculum document: www.mmc.nhs.uk/pages/foundation/Curriculum If you have not achieved these competencies, you will need to apply for the second year of the Foundation Programme, F2 (you are not eligible to apply for F1 if you have completed an internship). Currently there is no centralised application scheme for the stand-alone F2 posts. These will be advertised on the deanery websites and in the medical press in the spring. The majority of hospital posts are advertised in the BMJ and The Lancet. Both journals have websites: www.bmjcareers.com and www.thelancet.com The national on-line recruitment website for the NHS www.jobs.nhs.uk also carries some medical vacancies.Details of the Deaneries can be found on www.copmed.org.uk As you are not considering starting until 2008, I advise you to keep checking the MMC and MTAS websites for updated information. I hope this information is of assistance. Kind regardsJulia WardleNACPME

Evidence that you have attained Foundation competences.? If you have successfully completed the two-year Foundation training programme you will be able to have a copy of your Foundation certificate and supervisors report in your portfolio. ? If you successfully completed 12 months of UK SHO training in approved posts this is accepted by PMETB as having given you the Foundation competences. Letters of appointment and assessments confirming satisfactory completion of posts should be included in your portfolio.? If you have not completed a Foundation programme or 12 months of UK SHO training in approved posts, you need to provide evidence in your portfolio of attainment of the Foundation competences that are listed below. The CEM curriculum details how these General Medical Council (GMC) principles of ?Good Medical Practice? relate to the specialty of EM and may provide you with further ideas of evidence you might compile for your portfolio.
